PURPOSE:To evaluate phase-contrast magnetic resonance (MR) imaging for sizing of secundum atrial septal defects (ASDs) and inflow MR angiography for detection of associated venous anomalies in pediatric patients with inconclusive transthoracic echocardiographic (TTE) results.

Early diagnosis is the single most important step in the definitive management of the condition; otherwise, only conservative treatment can be offered. In this report, we present the case of a 20-year-old female patient diagnosed with Eisenmenger syndrome secondary to a large secundum ASD. The patient responded well to medical treatment.

The ostium primum ASD is the second most common type. An ASD is characterized by a defect in the interatrial septum that allows pulmonary venous return to pass from the left to the right atrium (i.e., left-to-right shunt), resulting in right atrial and right ventricular chamber dilation. There are four types of ASD. The first type is an ostium secundum defect. A more robust method for demonstrating a secundum ASD is to acquire phase-contrast images parallel to the septum at the level of the fossa ovalis. With this method, blood flow through the interatrial septum travels perpendicular to the imaging plane and easily detected. The secundum ASD is usually an isolated lesion, but it can be seen in association with other forms of congenital heart disease. The two common associations are mitral stenosis (Lutembacher syndrome) and absent radius (Holt–Oram syndrome).
